Life Coaching Agreement
We agree to enter a life coaching relationship, under the guidelines provided by the American Counseling Association Ethical Code. A copy is available to the client anytime.
As life coach, I agree to provide counseling services such as support, clarification, resource seeking, solution finding, goal setting, skill building, self esteem recovery, etc. As client, you agree to regard me as the assistant rather than the boss.
All information given to me is held in the strictest confidence, and will not be given to anyone without written permission. However, there are several exceptions: a client who intends suicide or harm to another, or is in a medical emergency, may require active intervention.
You decide how long the session goes. The first session is free. After that, there's a sliding scale with a minimun of $20 an hour, if you can afford that. You decide where you belong on the scale.
If you have to miss an appointment, you please let me know as far in advance as you can.
